What LeaveMD is

LeaveMD connects you with independent, licensed clinicians who evaluate whether medical leave certification is appropriate for your situation, and we handle the administrative work around that evaluation — scheduling, completed certification paperwork, and help responding to HR follow-ups.

By using this site or purchasing an evaluation or membership, you agree to these terms. If you don't agree, please don't use the service.

What LeaveMD is not

Not a guarantee. Certification is a clinical judgment made by a licensed clinician after evaluating you. Clinicians decline when certification isn't appropriate — that's what makes the service legitimate, and it's why the refund policy below exists. Purchasing an evaluation does not guarantee certification, and certification does not guarantee that your employer or a state program will approve your leave.

Not medical care beyond the evaluation. The telehealth evaluation is a real clinical visit, but LeaveMD is not your treating provider, is not an emergency service, and does not provide ongoing medical care, prescriptions, or treatment. Not legal advice. Information on this site about FMLA and state leave programs is general information, not legal advice, and using the service does not create an attorney–client relationship. Eligibility depends on your specific circumstances.

Eligibility to use the service

You must be at least 18 years old, located in the United States, and using the service for your own leave needs. You agree to provide accurate, truthful information in your intake and evaluation — clinician judgments and completed paperwork are only as good as the information you provide, and knowingly providing false information to obtain certification may have legal consequences for you.

Pricing, payment, and refunds

One-time evaluation: $159.99, charged when you book. If the clinician determines certification is not appropriate, you receive a full refund.

Membership: $49 per month for unlimited clinician consultations, certification paperwork, amendments, and HR follow-ups. Billed monthly; cancel anytime and billing stops at the end of the current period. Membership fees for periods already elapsed are not refunded.

All prices are in U.S. dollars. Payments are processed by Stripe. To request a refund under the policy above, email support@leavemd.com.

Your responsibilities

Attend the telehealth visit you schedule (or reschedule in advance), provide truthful information, and submit your completed paperwork to your employer yourself unless you explicitly ask us to help. Deadlines for FMLA and state programs are yours to meet — we work quickly, but we can't control your employer's or a state agency's timelines.

Intellectual property and acceptable use

The site, its content, and its branding belong to LeaveMD. You may use the site for its intended purpose. You may not scrape, copy, resell, probe, or interfere with the service, or use it to harass clinicians or staff.

Disclaimers and limitation of liability

The service is provided “as is.” To the maximum extent permitted by law, LeaveMD disclaims all warranties, express or implied, and is not liable for indirect, incidental, special, or consequential damages, or for employment decisions made by your employer. Our total liability for any claim relating to the service is limited to the amount you paid us in the twelve months before the claim arose.

Nothing in these terms limits liability that cannot be limited by law, and nothing changes the professional obligations a licensed clinician owes you during a clinical evaluation.

Changes and termination

We may update these terms; material changes will be reflected in the date at the top of this page. We may suspend or end service to anyone who violates these terms. You can stop using the service, or cancel a membership, at any time.

Governing law and disputes

These terms are governed by the laws of the State of Delaware, without regard to conflict-of-law rules. Before filing any claim, you agree to contact us at support@leavemd.com and give us 30 days to work it out — most issues are resolvable by talking to a human.
